The role:
You will be responsible for supporting the move on from a variety of Supported Housing by reviewing each case and identifying any barriers and problems preventing them from moving on. Furthermore, you will train and mentor your team in order to expand their knowledge and expertise around Housing Options, procedures and legislation. You will also be responsible for supporting the individual to complete all necessary paperwork and getting in contact with all relevant agencies.
Your responsibilities:
- Maximise and explore the housing options available whilst ensuring a planned and safe discharge.
- Act as an advocate for patients, such as challenging and following up instances of unsuccessful referrals or bad advice.
- Negotiate and influence across a wide range of services at all levels of seniority.
- Liaise with partners in mental health, adult social care, the third sector and statutory and non-statutory partners to make sure the appropriate support is in place at point of discharge.
